Next-day impacts of insomnia

Insufficient sleep has been associated with difficulty focusing, impaired memory, impaired decision-making, and slower reaction times. This underscores the importance of treating and managing insomnia for employability, safety, and quality of life. According to the DSM-5, the diagnosis of insomnia is based not only on nighttime sleep issues but also daytime impairment that can impact a patient’s academic, social or work life. In a recent study, 73% of insomnia patients complained of challenges falling asleep, and 84% experienced issues with daily functioning.



In clinical trials, patients taking daridorexant (Quviviq) 50 mg reported improvements in daytime sleepiness versus placebo as measured by the Insomnia Daytime Symptoms and Impacts Questionnaire (IDSIQ). This improvement started 1 week after the initial administration and continued to improve over time. Statistically significant nighttime improvements versus placebo were also seen in falling asleep faster, staying asleep longer, and increased total time asleep. These results could be promising since many sleep medications are associated with next-day hangover effects.



Why do you think that patients taking daridorexant 50 mg showed a statistically significant reduction in daytime sleepiness vs placebo? What factors may be contributing to this finding?

    Insomnia is associated with daytime sleepiness, fatigue and inability to focus,
    Also is associated with increased risk of MVA, especially for long distance drivers.
    There is increased symptoms of irritable mood and sad mood.

    Patients showed improvement quality of life, better sleep with no significant side effects .

    Insufficient sleep always causes daytime fatigue and sleepiness. Would be an option to treat idiopathic hypersomnia pts with this to see if sleepiness goes away.

    I have prescribed similar insomnia meds and seen then same positive results. The mechanism of action differs from other types so likely is the cause.

    Sleep is supposed to be revitalizing and refreshing experince to "recharge body physically and chemically " and subequent day functioning in being awake and efficient related to the Intrinsic effect of the hypnotic agent in resulting in the high quality sleep induction and maintenance and data i guess reflects on such efficacy of the Daridoxerant ! looking forward in using it !

    Having a new medication with a different mechanism of action is exciting
    I agree with the other MD who commented on the side effect profile of the older sleep drugs. Definitely deeper refreshing sleep helps with daytime fatigue.

    Good to have another option for insomnia. Would like to see how it compares to melatonin. No anticholinergic side effects is a benefit. Although there is reportedly no physical dependence, it is a controlled substance.

    Sleep disorders and interruption are associated with myriad disorders and have downstream deleterious effects including cardiovascular, neurologic/cognitive, and need to be addressed whether treating anxiety/depression, hypertension, metabolic syndrome. Serial testing should be used during treatment to demonstrate clinical improvement rather than only self-reported amelioration.

    Improved sleep duration and quality based on mechanism of action on wake cycle

    I believe that the duration and quality of sleep helped to improve daytime functioning especially if there is less nighttime awakening.

    Very likely due to improved sleep architecture - not just more sleep, but more truly restful sleep and more consistently as well.

    I bet the fact that they were sleeping better at night made them feel awake and refreshed during the day.

    Older sleep aid such as Ambien caused patients to sleepwalk, impaired memory and daytime hangover, poor reflex while performing daytime function such as driving. This drug Quviviq seemed to solve this problem, I would definitely prescribe for my older patients

    It appears to me that daridorexant improves sleep quality which, in turn, would dcrease daytime sleepiness. It is interesting there is less next day daytime sleepiness since the half-life of daridorexant is 8 hours

    Daridorexant improved Sleep quality and quantity hence improvement in daytime activities and better quality of life, productivity increases.

    Patients receiving Daridorexant showed a statistically significant reduction in daytime sleepiness likely due to better sleep efficiency associated with improvement in total hours of efficient sleep. Of course, knowing the number needed to treat would be an important statistic—that is, how many people would one need to treat with the medication before a single individual would benefit from it? Versus, the number needed to harm, eg. This would give even more data to support use of the agent that may be expensive or not easily covered by insurance.

    Improving sleep quality will have a notable impact on daytime sleepiness and fatigue, along with less anxiety over potential sleepless nights also improves quality of life. Patients are more able to follow improved sleep hygiene along with consistency in nighttime sleep hours.

    Promising as it seems to improve daytime functioning. Rule out possible placebo effect. Would still talk with pts about typical precautions such as with driving, combining it with other sedatives etc.
