Patient Background:

Ms. R is a 27-year-old female presenting with a 5-year history of chronic excessive daytime sleepiness (EDS), prolonged nighttime sleep (10–12 hours), and severe sleep inertia, sometimes described as “sleep drunkenness,” lasting 60–90 minutes each morning.

She frequently oversleeps alarms despite appropriate sleep hygiene. Symptoms significantly impair her ability to work as a nurse. She previously tried modafinil (partial benefit, discontinued due to headaches) and armodafinil (minimal benefit).PHQ-9: 8 (mild depressive symptoms).Family history includes a brother with excessive sleepiness.read more

No cataplexy. BMI: 22 kg/m². Non-smoker.

Assessment & diagnosis:

Overnight PSG shows no evidence of sleep apnea (AHI <1), normal REM latency, and no evidence of narcolepsy. Mean sleep latency on MSLT is 6 minutes without SOREMPs. ESS: 20/24. IHSS: 36.

Diagnosis: idiopathic hypersomnia (IH). The care team discusses treatment options and counseling strategies to address excessive daytime sleepiness, prolonged sleep duration, and impaired daytime functioning.

Counseling included discussion of medication safety, CNS depression risk, avoidance of alcohol and sedative coadministration, and impaired alertness during treatment initiation.
